At YEMS Outreach, we believe that everyone deserves to feel connected, valued, and involved in their community. Our Community Participation supports are designed to help NDIS participants build relationships, engage in social and recreational activities, and feel confident participating in the world around them.

We support people to explore their interests, try new things, and enjoy meaningful experiences — whether that’s attending a local event, joining a group activity, or learning new skills.

Overview

Community Participation is about more than just getting out of the house — it’s about feeling included, empowered, and supported to be yourself in social settings. We work with each participant to identify goals, preferences, and interests, then build a plan to help them connect with others and enjoy active, fulfilling lives.

Our support workers can assist with:

  • Attending social events or group outings
  • Participating in sport, arts, or hobby groups
  • Visiting friends or family
  • Building communication and social skills
  • Volunteering or work experience opportunities
  • Accessing local community services and programs
